About the Role : As a social worker-Healthy Steps specialist, you’ll work with a care team that includes a physician or pediatric nurse practitioner, and consult with families with babies and young children regarding developmental and behavioral concerns. You will be primarily based at our Mattina location, 300 Niagara St. in Buffalo, and will travel to other Neighborhood sites in Western New York as needed. Requirements

• Master’s degree in social work and a strong desire to work with an urban population; must possess a thorough knowledge of health issues affecting the population throughout the lifecycle.
• NYS Licensure - LMSW or LCSW
• Able to read, write and speak the English language.
• Excellent written and verbal communication skills

Nice To Haves

• A second language of Spanish preferred.

• Oversees and provides direct and team-based behavioral health care with supervision
• Facilitates care coordination and clinical collaboration with supervision
• Ensures quality care and risk mitigation with supervision
