About The Position

Walden Family Services, an established Foster/Adoption agency, is looking for a Full-Time social worker (bilingual preferred) to work from our Riverside Office to cover cases throughout Riverside and San Bernardino Counties. This worker will be responsible for working with families and youth in foster care. In addition to this they will be responsible for providing a continuum of care for the children who have been placed in Walden Family Services’ programs. The Social Worker will be responsible for working with the child, foster family, adoptive family, and natural family, significant others, placing agency and adjunct treatment persons. DESCRIPTION OF POSITION: The Social Worker will be responsible for working with the child, resource family, biological family, significant others, placing agency, and adjunct treatment persons to enable the child to attain his or her treatment goals.

Requirements

  • Masters level social worker required
  • Experience in working in child and adolescent services
  • Must have the ability to work effectively with others as a member of a treatment team.
  • Fingerprint and Child Abuse Index clearances from the Department of Justice.
  • Must meet all requirements set by Community Care Licensing pertaining to the hiring of employees by a licensed community care facility
  • Candidate must have a minimum of a master’s degree in social work or a closely related field, as defined in the State Department of Social Services, Community Care Licensing regulations.
  • Must possess valid California Driver’s License and have a good driving record.

Responsibilities

  • Be knowledgeable of the Social Worker Manual, Program Statement, Title XXII, and Agency policies and guidelines.
  • Conduct an intake meeting with the client, placement worker, natural parent, and resource parent, if all parties are available, when the child is placed with the Agency. Explain the program expectations and complete necessary intake paperwork, including the Appraisal Needs and Services Plan, or 30 day initial Treatment Team Review.
  • Following the Intake meeting, hold and create an initial treatment plan report within 30 days of placement, in coordination with other treatment team members...
  • Conduct a Treatment Team Review (TTR) on a quarterly basis to review progress made towards established goals following the TTR procedures. Prepare a quarterly report immediately following the TTR, and submit to the supervisor within 10 working days.
  • Provide weekly consultation to the treatment resource parent, in the resource home. Consultation will consist of discussion of child’s progress towards goals as defined in the Treatment Team Review, as well as teaching and discussion of therapeutic interventions, disciplinary techniques and resource parent training. Social Worker will provide and/or coordinate referrals for additional services as needed (i.e. medical, psychiatric, etc.)
  • Provide weekly support and case management to enable child to accomplish treatment goals. If a child has needs that require the services of an outside therapist for a particular issue, the Social Worker will work on other issues with the child in coordination with the outside therapist, for purposes of consistency and support of written treatment goals.
  • Document child’s weekly progress at home, school, and/or work, major incidents, visitation and other significant, collateral contacts.
  • Obtain approval from placement worker prior to allowing any contact between the child and others. Approve and facilitate all ongoing visitations with child.
  • Encourage resource parent to involve child in community activities and extra curricular activities.
  • Consult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed.
  • Meet with supervisor for a minimum of one hour weekly of individual supervision and case consultation.
  • Ensure that client files in the facility and office are complete and up-to-date.
  • Utilize KaleidaCare web-based system to maintain client documentation, including weekly contact notes, SIRs and personal documentation for the client.
  • Submit all reports and documentation to supervisor for final approval.
  • Complete monthly foster home inspection forms to ensure compliance with CCL regulations.
  • Facilitate resource parent and co-worker training, and support groups as directed by supervisor.
  • Complete initial permanency assessments and written reports for potential resource parents. Submit resource parent annual assessments for recertification and as needed updates to this assessment as required by Community Care Licensing.
  • Provide on-call crisis intervention. Perform the function of duty-worker at the office as requested.
  • Conduct monthly and quarterly foster home inspections and address compliance issues as needed.
  • When the Skills Trainer is working with a family and/or child on the Social Worker’s case load, the Social Worker will consult/supervise the Skills Trainer weekly regarding goals and/or progress. The Social Worker must first obtain approval from their supervisor to utilize the services of a Skills Trainer prior to scheduling an initial appointment.
