Requirements

  • Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.)
  • Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team

Nice To Haves

  • At least one year of supervised social work experience in a health care setting preferred but not required

Responsibilities

  • Admissions: Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
  • Care Planning: Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.
  • Discharge Planning: Coordinate follow-up services and provide resources to residents and families.
  • Documentation: Complete timely and accurate notes, assessments, and reports in compliance with regulations.
