Requirements

Bachelor’s Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.)
Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team

Nice To Haves

At least one year of supervised social work experience in a health care setting preferred but not required

Responsibilities

Admissions: Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.
Care Planning: Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.
Discharge Planning: Coordinate follow-up services and provide resources to residents and families.
Documentation: Complete timely and accurate notes, assessments, and reports in compliance with regulations.
