About The Position

ATC Healthcare Services is seeking a compassionate and dedicated Social Worker to join our team in Farmville, VA . The Social Worker will support individuals and families by providing counseling, care coordination, and access to community resources. This role focuses on improving clients’ overall well-being, addressing social and emotional challenges, and helping individuals achieve stability and independence.

Requirements

• Bachelor’s or Master’s degree in Social Work (BSW/MSW) or a related field
• Active state licensure (if required in Virginia)
• Previous experience in case management, counseling, or community support services preferred
• Strong communication, empathy, and problem-solving skills
• Ability to work effectively with individuals from diverse backgrounds

Responsibilities

• Conduct client assessments to identify social, emotional, and environmental needs
• Develop and implement individualized care and support plans
• Provide counseling and crisis intervention to individuals and families
• Connect clients with community resources , housing, healthcare services, and support programs
• Collaborate with healthcare providers, case managers, and community organizations
• Maintain accurate case documentation and reports in accordance with organizational policies
• Advocate for clients to ensure they receive appropriate services and support
• Monitor client progress and adjust care plans as necessary
