About The Position

This Social Worker as a member of the Interdisciplinary Group (IDG), provides medical social services and support to patients and their families. The social worker will provide all psychosocial interventions and referrals, assessments, supportive counseling, networking, and resource procurement. Job duties are performed with awareness and respect of the patient’s developmental stage, patient rights, and patient and family personal, cultural, and spiritual beliefs, according to specific program guidelines. Contributes to the smooth functioning of services, and advocates for patient/family needs with the team and community at large.

Requirements

  • Knowledge of psychosocial assessment, counseling, and case management techniques.
  • Familiarity with end-of-life care, grief, and bereavement support practices.
  • Understanding of cultural, spiritual, and developmental diversity in care.
  • Strong communication, documentation, and interpersonal skills.
  • Ability to navigate sensitive conversations regarding goals of care and transitions.
  • Competence in interdisciplinary collaboration and ethical decision-making.
  • Proficiency in basic computer and electronic record systems.
  • Minimum of 3 years of direct service experience in a healthcare or social services setting.
  • Experience in individual and group counseling and case management required.
  • Valid Florida driver’s license and active automobile insurance required.
  • Master’s Degree in Social Work from an accredited school required.
  • Medical clearance for communicable diseases required prior to patient contact.
  • Up-to-date immunizations including TB, Hepatitis, and Flu required.
  • Must follow NASW Standards of Practice for Palliative and End-of-Life Care.

Nice To Haves

  • Prior work with patients and families facing terminal illness preferred.

Responsibilities

  • Conduct psychosocial assessments addressing emotional, mental health, and environmental needs.
  • Provide therapeutic services including short-term counseling, crisis intervention, and bereavement support.
  • Assist with advance directives, funeral planning, and resource referrals.
  • Identify and coordinate access to internal and external resources.
  • Participate in Interdisciplinary Group (IDG) meetings and contribute to care planning.
  • Advocate for patient/family rights and support self-determination.
  • Evaluate response to psychosocial interventions and document outcomes.
  • Educate patients, families, and caregivers on psychosocial topics and coping strategies.
  • Maintain timely and accurate documentation according to policy.
  • Adhere to ethical and professional standards per NASW guidelines.
  • Participate in on-call rotation and respond to urgent needs as required.
  • Attend and contribute to internal and community-based meetings or trainings.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Perform other duties as assigned within the scope of practice.
