About The Position

At Qualicare Home Health, we are committed to delivering compassionate, evidence-based care that empowers patients to achieve their highest level of independence and well-being. We are currently seeking a skilled and compassionate Speech-Language Pathologist (SLP) to join our San Ramon-based team. In this role, you will evaluate and treat patients experiencing speech, language, cognitive, and swallowing disorders, helping them regain communication skills and improve overall quality of life in the comfort of their homes.

Requirements

  • Active, unrestricted California Speech-Language Pathology license
  • Master’s Degree in Speech-Language Pathology from an accredited program
  • Minimum 2 years of clinical experience (home health or adult rehabilitation preferred)
  • Current CPR certification
  • Strong communication, documentation, and patient education skills
  • Valid California driver’s license and reliable transportation

Nice To Haves

  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Experience with EMR systems (WellSky or similar) preferred

Responsibilities

  • Perform comprehensive speech, language, cognitive, and swallowing evaluations
  • Develop individualized treatment plans based on patient needs and clinical goals
  • Provide therapy interventions to improve articulation, fluency, cognitive-linguistic abilities, and swallowing safety
  • Educate patients and caregivers on communication strategies, swallowing safety, and home exercises
  • Collaborate closely with PTs, OTs, RNs, and caregivers as part of an interdisciplinary care team
  • Monitor patient progress and modify treatment plans as needed
  • Maintain accurate documentation in EMR (WellSky) in compliance with Medicare and agency standards
  • Communicate treatment updates and outcomes to physicians and care teams
