About The Position

Summit Home Care and Hospice is searching for an experienced and motivated Speech Therapist/Speech Language Pathologist to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care. Speech Therapist/Speech Language Pathologist: PRN Hours: As needed basis Pay: Flexible DOE (Based on a Per Visit Rate) Coverage Location: East Cincinnati (Mt. Orab, Georgetown, Bethel, Williamsburg) Travel: Required + Milage Reimbursement

Requirements

  • Possesses a master's or doctoral degree in speech-language pathology (SLP).
  • Must meet either of the following requirements: be licensed as a speech-language pathologist by the State in which the individual furnishes such service; or if the State does not license speech-language pathologists: must have successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience); performed not less than 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field; and successfully completed an approved national examination in SLP.
  • A minimum of one (1) year's clinical experience in Speech-Language Pathology is preferred.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements
  • A recent TB test is required.
  • Demonstrates good verbal and written communication, and organizational skills.

Responsibilities

  • The Speech Therapist/Speech-Language Pathologist is responsible for implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.
  • Conduct appropriate evaluations, assess home environment (as appropriate), identify equipment needs relative to speech/language function.
  • Identify functional speech defects and establish a plan of care to improve patient function.
  • Administer speech therapy program utilizing specialized therapeutic techniques and/or equipment.
  • Recommend communication devices/aids as indicated.
  • Work with the referring physician and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participate in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.
  • Prepare and submit notes per organization policy.
  • Maintain clinical competency in speech-language pathology practice and theory.
  • Identify patient and family/caregiver needs for other services and refer as appropriate.
  • Instruct patient, family/caregiver and other Organization health care personnel in the patient's treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech-language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech-language pathology services.
