About The Position

Your Role The Community Programs team delivers high-touch engagement with members and community-based organizations to remove barriers to care, enhance population health initiatives, and advance health equity. The Sr. Manager of Community Programs will lead community outreach initiatives that drive membership growth and retention, oversee the Blue Shield Promise Information Resource Centers (IRCs) in San Diego County, and support the management of grassroots marketing sponsorships and community investments. This role provides strategic and operational oversight of community and social programs aligned with Blue Shield Promise’s goals for health equity, membership, population health, quality, member experience, and service delivery. This role will report to the Sr. Director of Community Programs. Requirements

  • Requires a college degree or equivalent experience
  • Requires at least 10 years of prior relevant experience, including 4 years of people management experience
  • Requires at least 5 years of Medi-Cal experience
  • Requires experience designing and operationalizing care navigation and outreach programs
  • Proven ability to build and sustain partnerships with community-based organizations, schools, health systems, and government agencies to support coordinated referrals and service delivery
  • Must have excellent oral and written communication skills with ability to influence cross-functional teams, including interfacing with key internal and external stakeholders, health plan members, providers, and the public

Responsibilities

  • Be responsible for a singular team, department, or functional area; scope of oversight includes planning, cost management and methods
  • Collaborate with cross-functional leaders to manage the day-to-day operations and management of the IRCs to ensure consistent service delivery
  • Establish and manage the Community Connector Health Advocate team to support individual member needs through collaboration with internal departments, program referrals, information sharing and linkage to community resources
  • Develop, implement, and evaluate community-centered programs that drive membership growth and retention and improve care navigation, quality and cost-of-healthcare
  • Oversee improvements to processes, policies, and procedures to track progress against strategic objectives to ultimately achieve business goals
  • Manage stakeholder relationships by communicating and building rapport with community organizations, vendors, and external partners to advance program goals
  • Develop and coordinate local marketing and outreach campaigns, volunteer engagement, and event-based promotion to build community presence
  • Proactively build and maintain relationships with health systems, community-based organizations, local agencies, and internal partners to expand program reach and resources
  • Provide developmental and training opportunities for team members
