About The Position

Position Overview The SWAT Registered Nurse is an autonomous, highly skilled clinician who supports inpatient units through early identification and intervention for deteriorating patients. This role rounds proactively on high risk individuals—such as those with elevated Deterioration Index scores or Sepsis Alerts—and responds to CPR, Rapid Response, and Inpatient Stroke Team activations to enhance patient safety and prevent clinical decline. The SWAT RN delivers expert assessment, coordination, and evaluation of care; provides patient and family education; safely administers medications; and supports unit efficiency through effective resource use. The RN mentors new staff, participates in shared governance and quality improvement, integrates evidence based practice, and may assume Charge Nurse responsibilities when assigned. Through the diagnosis and treatment of human responses and advocacy for individuals and families, the SWAT RN maintains full accountability to the New York State Nurse Practice Act, United Health Services policies, the ANA Scope and Standards of Practice, the ANA Code of Ethics, and the Bill of Patient Rights, promoting a caring, healing, and patient centered environment.

Requirements

  • Bachelor of Science degree in Nursing
  • At least five (5) years progressive and relevant clinical and/or Nursing education experience.
  • Minimum 2 years of experience in Critical Care/ Emergency Department
  • Current New York State Registered Nurse License.
  • Completion of the Basic Coronary Care Course during orientation period
  • Medication Administration/Moderate Sedation during orientation period
  • Drain and Suture Removal competency during orientation period.
  • Stroke Education/Apex Module completion within 4 months of hire
  • Helipad Safety within first year of hire
  • ACLS within 60 days of hire
  • PALS within 60 days of hire

Responsibilities

  • Participates in response to Cardiopulmonary Resuscitation Team, Rapid Response Team, and Inpatient Stroke Response Team activations.
  • Provides support to inpatient units for patients with high Deterioration Index or Sepsis Alerts through active rounding and assessment of identified high risk patients.
  • Assesses, plans, delegates, coordinates, implements, and evaluates nursing care in managing a group of patients, serving as a primary nurse and/or accountable nurse, when designated by unit assignment and consideration of factors related to safety, effectiveness, competency, cost, and impact.
  • Educates patients and family in health promotion and disease management using available resources.
  • Utilizes a working knowledge of pharmacology and safely administers medications according to UHS Hospitals policy.
  • Shares responsibility for productivity and effective resources utilization including staffing, time management, supplies and services.
  • Complies with all nursing practice standards, regulatory requirements, and UHS policies.
  • Actively participates in and supports the Nursing Shared Governance Model by supporting the Unit Council, participating in improvement initiatives, and contributing to peer review practices.
  • Actively engages in professional development, continuing nursing education, and completing all mandatory education requirements.
  • Integrates nursing research and current evidence into practice.
  • Provides a supportive learning environment for new employees and students.
  • Performs duties related to the Charge Nurse or Shift Coordinator role when assigned.
  • Provides supervision and support to Graduate Nurses and new RN hires/transfers to promote delivery of safe patient care.
  • Complies with all documentation requirements and educates accordingly.
  • Complies with hospital policies and procedures and educates accordingly.
  • Reinforces education around key quality/safety alerts that have direct impact on patient outcomes.
