About The Position

This job opportunity is for a Per Diem Staff Pharmacist who will play a key role in supporting the daily operations of DVCH’s Pharmacy Services on an as-needed basis. The position focuses on safely and accurately dispensing medications, reviewing prescriptions for appropriateness, and providing patient-centered counseling to ensure proper medication use. This position is ideal for pharmacists who want flexibility in their schedule while maintaining their clinical skills and contributing to patient care. The candidate will need to be highly reliable and flexible, as they will be responsible for covering shifts during other pharmacists’ call-outs, vacations, and other scheduling gaps to ensure continuous pharmacy operations.

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an ACPE-accredited school of pharmacy or B.S. in Pharmacy degree from a school of pharmacy accredited by the Accreditation Council for Pharmacy Education.
  • Current, active, and unrestricted Pharmacist license in the Commonwealth of Pennsylvania.
  • CPR certification preferred.
  • Pharmacy experience in retail, ambulatory care, hospital outpatient, or FQHC/community health setting preferred.
  • Experience working with multicultural and multilingual populations preferred.
  • Familiarity with 340B programs and Federally Qualified Health Center (FQHC) pharmacy operations strongly desired.
  • PHYSICAL DEMANDS: Prolonged periods of sitting or standing and computer use during scheduled shifts. Lift up to 25-pound boxes may be required. Travel between health centers may be required.
  • WORK ENVIRONMENT: Work is primarily performed in a busy multi-cultural ambulatory care setting. There is routine exposure to infectious disease, blood borne pathogens and other conditions common in a clinical environment.

Responsibilities

  • Report to work as scheduled and maintain reliable attendance for assigned shifts.
  • Dispense medications safely and accurately in compliance with federal, state, and organizational regulations
  • Review and verify prescriptions for accuracy, appropriate dosing, drug interactions, and contraindications
  • Screen and validate physician orders for correct medication, dosage form, and therapy appropriateness
  • Provide patient counseling on medication use, side effects, adherence, and storage using clear, culturally sensitive communication
  • Collaborate with physicians, nurses, pharmacy technicians, and other healthcare staff to coordinate patient care
  • Maintain accurate documentation in pharmacy systems and the electronic health record (EHR)
  • Ensure compliance with pharmacy regulations, organizational policies, and HIPAA patient privacy standards
  • Maintain a high level of patient care, teamwork and professionalism
