We are currently seeking a Substance Use Medical Case Manager who will be responsible for coordinating medical and behavioral health care for individuals receiving substance use treatment services. This role ensures continuity of care, facilitates communication among providers, and supports clients in accessing necessary medical, psychiatric, and social resources. The position requires knowledge of ASAM criteria, DMAS regulations, and integrated care principles.
Requirements
- RN, LPN, CNA, CMA, or other medical training
- knowledge of ASAM criteria, DMAS regulations, and integrated care principles
Nice To Haves
- experience with SUD and mental health preferred
