About The Position

The Success Coach is primarily responsible for the engagement, assessment to determine needs, goal development and creation of extended learning opportunities to help L.E.A.P. High Afterschool students successfully transition to high school (9th through 12th Grades), develop meaningful relationships with their families, school and community, stay on track for graduation and begin to develop a plan for post- secondary success. Essential Functions: Conducts a complete intake and registration of students within stipulated Provides required support to help students transition to high school, utilize available resources and personnel to ensure a successful high school experience, to graduate on Works with agencies and service providers to facilitate case coordination and information sharing. Refers youth and their families to appropriate school and community agencies or counseling agencies for social and educational Maintains case records and provides assistance with appropriate documentation. Assists with prevention/education curriculum development and Works with guidance to coordinate, prepare and maintain required charting to help provide direction and documentation to proactively address challenges in a timely and thorough Adheres to all confidentiality requirements and Plans, initiates and monitors innovative programs for youth and their family's e.g. financial literacy, study skills, time management Works with culturally diverse students, families, and staff. May provide some direct supervision to interns and volunteers. Adheres to the Child Abuse & Neglect Reporting requirements, which mandate by law all childcare personnel to report suspicions of child abuse, neglect, or abandonment. The YMCA requires employees to follow the internal reporting procedures with the assistance of the Program Managers. Ensures that the proper staff to child ratios are maintained at all times and reports discrepancies to the Site Supervisor or Program Manager as needed. Requirements

  • Bachelor 's degree (A.) from a four-year college or university and a major in social work or a relate d field.
  • One (1) to Two (2) years' experience in working with adolescents in an educational, criminal justice, or human services setting require d.
