About The Position

The Supervisor of Transition Care will oversee a team of Transition Care Specialists, ensuring that client care is delivered effectively and efficiently. The Transition Care program plays a critical role in supporting individuals transitioning between levels of care in the mental health and substance use treatment continuum. This position ensures seamless coordination of services, promotes continuity of care, and addresses barriers to recovery. The coordinator acts as a bridge between clients, treatment providers, community resources, and support systems, fostering client-centered recovery plans and maintaining strong collaboration across multidisciplinary teams in both the inpatient and outpatient settings. The Transitional Care Supervisor will work directly with community partners and stakeholders to foster positive and collaborative relationships. The team is expected to work collaboratively to facilitate seamless transitions across the continuum of care from the inpatient environment to ambulatory care services and various community agencies ensuring the needs of the individual are met.

Requirements

  • Bachelor’s degree in Social Work, Nursing, Healthcare Administration, or related field (Master’s degree preferred).
  • 5+ years of experience in case management, with at least 2 years in a supervisory or leadership role preferred.
  • Strong understanding of case management principles, best practices, and healthcare systems.
  • Excellent leadership, communication, and organizational skills.
  • Ability to work collaboratively in a team environment and manage multiple priorities.
  • Strong problem-solving skills with a client-centered approach.
  • Ability to handle sensitive situations with confidentiality and professionalism.

Nice To Haves

  • Master’s degree preferred

Responsibilities

  • Supervise and support a team of case managers, providing guidance, mentorship, and performance evaluations.
  • Develop and implement policies and procedures to ensure quality care.
  • Ensure compliance with all regulatory and accreditation standards.
  • Collaborate with multidisciplinary teams to coordinate services and develop individualized care plans for clients.
  • Monitor and report on key performance indicators, case load management, and program effectiveness.
  • Provide conflict resolution and problem-solving support to care specialists and participants.
  • Maintain accurate records and oversee the documentation process to ensure all case files meet required standards.
  • Participate in recruitment, onboarding, and training of new transitional care staff.
  • Serve as a liaison between staff and upper management, advocating for team needs and communicating policy updates.
  • Ensure timely and efficient service delivery, while addressing barriers to care.
  • Conduct regular team meetings, provide updates on policies, and foster a positive team culture.
  • Maintain a limited caseload of participants enrolled in the program.
