POSITION SUMMARY: To ensure staff productivity and efficiency. To develop and promote care coordination for social service referrals between the Valley Health System entities to improve patient outcomes. To provide program development for the VHC volunteer, bereavement and Journeys programs. Assists in on-boarding, mentoring and training of the VHC social service department new hires in coordination with the VHC education department. To provide education to VHC staff and community partners about mental and social determinants of health awareness. To screen, assess, and evaluate patients and their families to identify psychosocial and discharge planning needs and develop appropriate interventions. To educate, counsel, and refer as indicated. To plan and communicate effectively with multi-disciplinary team and community resources.
