About The Position

Under the direct supervision of the Supported Living Services Director, the employee in this position is responsible for the completion of various trainings that will ultimately result in the independence of individuals served in their living situations. This position requires the employee to work without close supervision, while keeping good communication with the Supported Living Service Director. Responsible for quality assurance of client money accounts via paper and electronic ledgers. Responsible in assuring staff meet regulatory compliance, following all rules, laws, and policies related to client expenditures and maintenance of funds.

Requirements

  • Pass a level II DCF background check
  • Knowledge of regulatory standards and compliance requirements.
  • Freedom from illegal use of drugs.
  • Freedom from use and effects of drugs and alcohol in the workplace.
  • Bachelors in Human Services or related field, (Education, Social Services, Psychology, etc.) is required
  • Employment experience can substitute for the required education on a year to year basis.
  • Demonstrates willingness to try new tasks; generates new ideas for change; evaluates and recognizes priorities; selects effective team members, challenges others to learn, keeps current and integrates new information, communicates and models organization values, fosters high performance; recognizes need for and provides adequate resources.
  • Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs.
  • Ability to define and solve problems, collect data, establish facts and draw valid conclusions. Ability to deal with several abstract and concrete variables.
  • The employee is frequently required to stand and walk: use hands or fingers; reach with hands and arms. The employee is occasionally required to sit, climb or balance and stoop or kneel. Specific vision abilities required by this job include close vision, distance vision and peripheral vision. Pushing, pulling, and lifting boxes up to 15 lbs will occasionally be required.

Responsibilities

  • Establishes a general responsibility for Residents
  • Verbal and non-verbal interactions with Residents reflect caring and respect
  • Promotes individual rights
  • Communicates with consultants as needed in effort to provide care for residents
  • Communicates closely with service providers.
  • Establishes and Monitors recreation activities per IPP goals
  • Conducts and oversees assessments to implement and revise goals for residents
  • Supervises collection of baselines for programming efforts
  • Maintains program modifications as needed
  • Oversees teaching and training of staff and residents
  • Provides 1:1 and group teaching and training as needed
  • Creates the implementation programming
  • Creates schedule formedical care provided to those assigned to their caseload
  • Conducts ongoing training of Operation Managers
  • Trains Operation Managers on program modifications
  • Attends required meetings such as Admission, Discharge Planning and IDT meetings
  • Writes and prepares necessary correspondence, reports, assessments and summaries as required by regulatory standards (e.g., Delmarva, APD, AHCA)
  • Available by phone, or other facility approved means, with a reasonable response time
  • Reports to immediate supervisor as needed
  • Additional duties as assigned by immediate supervisor
  • Participates in the department budget process (monitoring time-worked)

  • Attends in-service and education programs; attends continuing education required for maintenance of professional certification or licensure.
