About the position

The Care Coordination Program focuses on prompt linkage to HIV care, ongoing retention in care, optimal treatment adherence, viral load suppression and reduced HIV-related mortality. The programs in this department work with people living with HIV/AIDS to improve overall health and establish self-management through case management, peer navigation, and health education services. The Supportive Counselors will play a dynamic role in enhancing these services by providing direct support and counseling services to clients by conducting assessments, developing individualized care plans, and providing ongoing support.

Responsibilities

  • Conduct comprehensive assessments and intakes to evaluate for client eligibility and goals
  • Develop individualized care plans to help clients stay engaged in their own care
  • Provide one-on-one supportive counseling sessions
  • Promote stability and support by addressing goals as described in the service plan
  • Conduct group supportive counseling to address service plan goals and enhance mutual support between clients
  • Participate in outreach activities to engage new clients and inform the community of available services
  • Collaborate with other CHASI programs and external providers to make culturally appropriate linkages and referrals to help achieve client goals; perform follow up to ensure a 'warm handoff'
  • Complete all progress notes and other reporting as required by internal practices and funders in a timely manner
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values
  • Other duties as assigned

Requirements

  • Master’s degree in mental health counseling, social work, or other related degree required
  • At least 2 years of experience providing case management or counseling services to people living with HIV/AIDS or other chronic illness or related population
  • Demonstrated experience with and/or understanding of providing evidence-based, client-centered, trauma-informed, culturally appropriate interventions rooted in harm reduction principals
