The Supportive Housing Manager (Empire State Supportive Housing Initiative) is responsible for overseeing the day-to-day operations of services delivered at The Gardens, 270 East Avenue or St. Leo Senior Housing. ESSENTIAL JOB FUNCTIONS Manage Service Coordinators to ensure that participant’s service plans and health and wellness assessments are current as per Department of Health guidelines. Conduct weekly high risk resident meetings with Service Coordinators. Provide training and orientation for newly hired staff to familiarize them with policies and procedures for the Supportive Housing Program and those of ESLC. Implement ESLC’s professional conduct expectations that result in a positive community image and awareness of ESLC and the services that are provided. Lead and collaborate with health and wellness coordinator or designee to facilitate programming that will enrich the lives of the ESSHI participants. Perform service coordination duties as needed. Lead, manage and supervise Health and Wellness Coordinator, Health and Wellness Assistant, Service Coordinators and Driver where applicable staffing patterns requires. Perform outreach to community referral agencies to inform of the program and to obtain referrals. Schedule and conduct apartment tours for individuals interested in program enrollment. Intake potential Supportive Housing residents and enroll them into the Supportive Housing program if appropriate. Work with outside vendors to coordinate services based on the needs of the Supportive Housing participants. Ensure Supportive Housing Participants receive quality services with high satisfaction through regular check-ins with the Participants. Conduct monthly staff meetings and staff supervisions (frequency dependent on employee needs) to ensure communication occurs and to hold employees accountable for stated goals and job expectations. Conduct quarterly Community Meetings to ensure Participants needs are being met and have a voice to be heard Maintain staff schedule and time off request Monitor, review and approval of timecards Manage, review and approve credit card purchases and allocations. Educate partners and residents of the ESLC Neighborhood Program. Responsible for a vibrant and successful Neighborhood Program at the Supportive Housing Program location. Ensure all funder guidelines are followed. Ensure grant targets and deliverables are met. Actively manage the Supportive Housing program site budget to include meeting budget targets and adapt as needed Review and approve appropriate invoices and expenses for the program site Work with property management company to ensure Participant experience standards are met Create new policies in collaboration with the Vice President of Community Services. Provide continuous feedback, including coaching to promote a high level of employee engagement and overall staff satisfaction. Complete annual performance evaluations. Collaborate with other ESLC team members as needed. Perform other necessary duties as required. Employee may perform other related duties as assigned to meet the on-going needs of the organization. The Supportive Housing Manager at St. Leo will have different job duties due to only serving Participants in 25 apartments: The Manager will carry a small caseload as this program has the equivalent of 1.0 FTE in Service Coordinators while the other programs due the number of Participants served being larger. The Health and Wellness and Neighborhood program is operated in conjunction with the Director of Community Dementia Services, and the program does not directly staff a Health and Wellness Coordinator (as the other Supportive Housing programs do). The Director of Community Dementia Services is responsible for ensuring there is a monthly Health and Wellness Calendar. There may be occasions that the Manager will be required to provide direct assistance with a Health and Wellness Program in collaboration with the Director of Community Dementia Services. This collaboration should result in less oversight and involvement in the Health and Wellness and Neighborhood Program activities and events.
