About The Position

The System Diabetes Education Coordinator is directly responsible for the implementation of developing, implementing, and evaluating the diabetes education program for patients and their families throughout all ARH communities. The program delivers comprehensive and seamless services that bridge the gap and integrate clinical and self-management aspects of diabetes care. The Diabetes Education Program Coordinator leads the interprofessional team that provides collaborative, comprehensive and person-centered care, and education conducive to behavior change and improved quality of life across the lifespan. The Diabetes Education Program Coordinator supervises the quality of education provided by diabetes educators and conducts yearly reviews of employees conducting diabetes education.

Requirements

  • Associate degree in nursing, dietetics/nutrition/pharmacy
  • Valid license based on location of practice as a Dietitian (must be registered by the Commission on Dietetic Registration), or Registered Nurse, or Registered Pharmacist
  • Licensed Diabetes Educator, Certified Diabetes Care and Education Specialist or BC-ADM credential preferred/eligible. If not already Diabetes Care and Education Specialist or BC-ADM, will sit for the certification examination within 24 months of hire date
  • Excellent verbal and written communication skills
  • Basic computer skills including use of Microsoft Office
  • Highly organized and strong attention to detail
  • Strong problem solving and critical thinking skills
  • Time management skills required with ability to prioritize and manage multiple projects and tasks simultaneously
  • Must be able to work with a diverse patient population and have exceptional customer service skills

Nice To Haves

  • Diabetes self-management education and support experience preferred
  • Has working knowledge of diabetes technology

Responsibilities

  • Oversight of Diabetes Self-Management Education (DSMES) Services
  • Implements the standards of DSMES
  • Ensures services are evidence-based
  • Facilitates development and communication of effective program policies and procedures to employees within the diabetes program
  • Ensures ongoing service evaluation and continuous quality improvement plan is reviewed at least annually
  • Provides ongoing support and education to clinical staff to achieve an excellent understanding of diabetes management
  • Ensures department aligns with and supports organizational policies
  • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, and self-management of diabetes into clinical practice.
  • Supervision of Educators
  • Appraises performance of Diabetes Educators (including self) to identify areas of strength and areas for improvement and to develop a plan for improvement and growth
  • Participates in peer review process to evaluate performance of other professional instructional staff
  • Coordinator and appropriate staff maintain 15 hours of continuing education annually specific to diabetes, diabetes related topics and behavior change and self-management education strategies
  • Contributes to, and participates in, a continuous quality improvement process to measure DSMES program and to identify and address opportunities for improvement
  • Assures sufficient staffing, appropriate work assignments and directs training and education of staff
  • Instruction of Program Participants
  • Collaboratively develops educational goals, learning objectives and a plan for educational content and teaching methods with DSMES program participants
  • Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions
  • Partners with individuals to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes and cardiometabolic conditions across the lifespan
  • Applies business principles, systems practice, and population health management to support achievement of reduced costs, better outcomes, improved experience, and improved work life for healthcare providers
  • Applies the AADE7 self-care behaviors to educate on and initiate behavior change
  • Engages in lifelong learning and serves as a role model of professionalism
  • Provides quality diabetes self-management education in individual and group settings based on assessed needs
  • Utilizes appropriate teaching techniques that are sensitive to the culture and learning preferences of the person with diabetes
  • Follows the Association of Diabetes Care and Education Specialists (ADCES) requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines
  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient’ assessed needs and goals and promotes successful self-management
  • Collaboratively develops an individualized follow-up plan with each program participant
  • Documents all individual contacts/visits in the Electronic Health Record and outcomes data base according to the guidelines in a timely manner
  • Evaluates effectiveness of educational services provided by measuring attainment of learning objectives
  • Conducts a follow-up assessment upon completion of DSMES program services
  • Collaborates, advocates, and confers other members of the diabetes care team in developing person-centered diabetes plans
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services
  • Identifies when a program participant’s needs are outside the scope of the instructor’s practice and expertise and arranges for additional services to meet needs
  • Collaborates with community partners to establish and maintain on-going support options
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards and safety standards; complies with governmental and accreditation regulations
  • Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate
