About The Position

POSITION SUMMARY: The Targeted Case Manager (TCM) will provide case management services to individuals in their assigned program of severe mental illness (SMI), substance use (SUD), and/or serious emotional disturbance (SED). Position consists of resource acquisition, linking to services, coordinating care for physical and mental health needs and monitoring changing needs of participant. DUTIES AND RESPONSIBILITIES: Assess client and family strengths and needs. Link client and/or family to appropriate community resources. Assist client/family with advocacy efforts with other community services. Maintain appropriate contact with client and/or family, to include a minimum of 4 contacts a month including 2 face to face. Collaborate with Lifeskills staff and external providers to assist in client’s recovery from mental illness and/or substance use. Assure client choice and access to community resources. Provide services in the clinic and the community (i.e. homes, school, courts, DCBS, etc). Assist individual to engage in activities that will provide opportunities for developing meaningful roles in his/her community (e.g. volunteer, employee, member, customer, patron etc.) Use personal vehicle to transport individuals to and from various community settings of their choice (must maintain current license and vehicle insurance). Conduct oneself morally, and ethically, serving as a positive role model. Provide billable targeted case management services to 23- 25 clients per month or appropriate to program. May fill in from time to time during the absence of other staff, as assigned by supervisor. Attend individual and group supervision as scheduled. Employee is responsible for timely documentation of services provided. Other duties as assigned/required. QualificationsQUALIFICATIONS: Requires a Bachelor of Arts or Science Degree in Psychology, Social Work, or behavioral science with one year of post graduate experience working with individuals with mental health, SUD, or SED. Must have reliable transportation with driver’s license and auto insurance as must be able to transport clients. Must possess strong written and verbal skills JOB REQUIREMENTS / EXPECTATIONS: Ability to sit for extended time for meetings, documentation, and clinic services. Ability to stand, walk, bend, and move within clinic and community settings. Must have car insurance and be able to transport clients for appointments and community engagement. Ability travel across regional sites when needed. Must maintain confidentiality and adhere to all regulatory and ethical standards. Requirements
