About The Position

Thresholds is seeking a Team Leader for Wellness West — a pivotal role supporting individuals living with serious mental illness and substance use disorders as they navigate hospital transitions, crises, and the path to long‑term stability. This is an opportunity to guide a multidisciplinary team, shape innovative care coordination on Chicago’s West Side, and directly impact health equity for some of the city’s most underserved communities. As the Team Leader, you will provide both clinical and operational leadership for a team including a Behavioral Health Care Manager, Community Health Worker and a Peer Support Specialist within a Community Mental Health Center (CMHC) setting. You’ll ensure members receive support through hospitalizations, timely coordination with emergency rooms, intensive community case management, and connections to mental health treatment. You will be at the center of critical moments—helping individuals stabilize after crisis, connecting them with care, and strengthening their ability to remain safely in the community.

Requirements

  • Passion for working in emergency rooms and community settings
  • Familiarity with higher‑acuity presentations
  • Ability to conduct screenings and assessments
  • Adaptability to staff and client needs
  • Superb time management and organizational skills
  • High school diploma or GED certificate and 5 years of supervised clinical experience
  • Bachelor’s degree in Psychology, Social Work, or related field
  • Bachelor’s degree in an unrelated field and 2 years of supervised clinical experience

Nice To Haves

  • Master’s degree in Psychology, Social Work, or related field preferred
