About The Position

Bill Wilson Center provides services to children, youth, young adults, and families in Santa Cruz County and Santa Clara County. Additionally, we reach clients through our street outreach and crisis line programs. Bill Wilson Center programs focus on housing, mental health care, supportive services, and advocacy. Under the direction of the Program Manager, the Case Manager provides intensive, goal-oriented support to participants navigating the transition from temporary to permanent housing in Santa Cruz County while also using the appropriate Santa Clara County referral systems when necessary. This role focuses on stabilizing households through education, employment, and parenting support, while utilizing 'Housing First' principles to remove barriers to independent living and long-term tenancy.

Requirements

  • (B.A.) in Social Work, Sociology, Psychology or related field plus 2+ years of relevant case management experience and/or training; 3+ years of on field experience, or equivalent combination of education and work experience.
  • Must be efficient and organized.
  • Must be able to communicate effectively, get along with coworkers and management and deal effectively and professionally under pressure.
  • Excellent interpersonal, organizational and communication skills; integrity; respect for confidentiality; ability to work with diverse populations, including persons with various disabilities.
  • Ability to analyze and prioritize complex situations, policies and procedures, laws and regulations and exercise good judgment in the course of completing required duties.
  • Regular attendance and punctuality are required.
  • Comprehend and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Computer skills such as word processing, spreadsheets, email management, internet navigation,and operating system navigation required.
  • Excellent communication skills required.
  • Driving is a requirement of the job (valid California driver's license with access to a personal vehicle that can be used for company business and current personal automobile insurance).
  • Fingerprinting/background check as required by the Department of Justice and/or Title XXII regulations.
  • Able to remain stationary for extended periods of time.
  • Ability to move around the property, office, and stairs.
  • Ability to move items up to 15lbs when necessary.

Nice To Haves

  • Preferred experience working with homeless clients.
  • Bi-lingual English/Spanish speakers are highly desired.

Responsibilities

  • Conduct comprehensive intake and assessment services to determine eligibility and immediate needs.
  • Partner with clients to develop and implement a goal-oriented housing stability plan, focusing on education, employment, and independent living skills.
  • Provide supportive, person-centered support to assist participants in overcoming barriers such as past domestic violence, substance use, or mental health challenges.
  • Conduct weekly or bi-weekly face-to-face meetings (in-office and home visits) to monitor progress toward self-sufficiency and housing readiness.
  • Facilitate the development of independent living skills, including budgeting, nutrition, and household management.
  • Coordinate with Coordinated Entry for referrals to maximize housing opportunities.
  • Regularly monitor housing conditions to ensure safety, cleanliness, and habitability standards are met
  • Conduct routine inspections of chores and home order; report any facility issues to the Manager or Facilities Manager immediately
  • Partner closely with the Housing Specialist to transition participants from transitional units into permanent, market-rate, or subsidized housing within the HUD-mandated timelines.
  • Utilize Santa Cruz community resources to meet the diverse needs of young parents.
  • Work closely with outside agencies to ensure a high quality of wraparound care for both the young adults and their children.
  • Assist young adults with locating appropriate child care resources and making arrangements for the proper care of their children.
  • Provide immediate crisis intervention as needed to maintain a safe and supportive environment.
  • Participate in a rotating after-hours and weekend on-call schedule to support residents during emergencies.
  • Safely transport clients to essential appointments and meetings when necessary.
  • Maintain real-time, accurate data entry into AWARDS and HMIS to ensure program compliance and funding eligibility.
  • Provide referral assistance or linkage to legal services, health services, drug and alcohol services and mental health services.
  • Utilize community resources to meet the needs of young adults in the program.
  • Report maintenance issues to manager or Facilities Manager immediately.
  • Participate in Quality Assurance/Quality Improvement activities.
  • Follow agency protocols and procedures regarding accounting and financial requests ensuring all expenses are approved and have back up documentation..
  • Participate in ensuring a safe work environment and proactively report safety issues to management immediately.
  • Regular attendance is a requirement of the position
