About The Position

This position coordinates and oversees statewide transition services for adults and medically complex youth aging into a adult Long-Term Services and Supports (LTSS) managed by the Office of Aging and Disability Services (OADS). This includes transitions from nursing facilities, hospitals, and other care settings, as well as transitions between Home and Community Based Service (HCBS) programs when individuals’ service needs change, requiring coordination across Medicaid and State funded programs. The position serves as the Project Director for Maine’s Money Follows the Person (MFP) demonstration grant, otherwise known as Homeward Bound. The MFP program supports individuals who have resided in nursing facilities and wish to transition into targeted 1915(c) waiver programs. Responsibilities include direct supervision and development of five Transition Coordination Specialists and a Data and Quality Analyst; program-level grant and contract oversight; fiscal monitoring; reporting; quality assurance; and compliance with state and federal requirements. The role works closely with DHHS partners, external providers, facilities, and federal partners, including regular engagement with the Centers for Medicare and Medicaid Services (CMS), to ensure effective, person-centered, and timely transitions into HCBS programs. The position also serves as a subject matter expert for complex transition cases across the adult LTSS continuum.

Requirements

  • A Bachelor’s Degree in Public Health or Health Sciences, Health Education, one of the Behavioral Sciences, Public Administration, or Education -AND- three (3) years’ experience in health program planning, development, implementation, and evaluation -OR- an equivalent seven (7) year combination of education, training, and experience.
  • Demonstrated experience supervising professional staff; managing complex health or human services programs; overseeing and managing complex budgets; and administering grants and contracts, including fiscal monitoring, compliance, and state and federal reporting requirements.
  • Experience supporting older adults and adults with physical disabilities within Medicaid-funded Adult LTSS delivered through HCBS, including State Plan services and 1915(c) waivers; interpreting and applying applicable laws, rules, and policies; and working collaboratively across state agencies, providers, and federal partners to ensure consistent practice and quality service delivery.
  • Proven ability to use data and quality improvement methods to strengthen HCBS service delivery and program outcomes, including development and application of quality control measures; experience supervising data and quality functions; and proficiency using Microsoft 365 and SharePoint-based systems to manage program operations, reporting, and dissemination of information.

Responsibilities

  • Provide direct supervision, coaching, and performance management of MFP Services Coordination Specialists and a Data and Quality Analyst, ensuring consistent, person-centered practice and compliance with member rights, health, and safety requirements within adult LTSS HCBS programs.
  • Lead statewide coordination of transitions for older adults, adults with physical disabilities, and medically needy youth transitioning from nursing facilities, hospitals, or other care settings into Adult LTSS delivered through HCBS, including Medicaid State Plan services, 1915(c) waiver programs, and state-funded home-based supports.
  • Oversee Maine’s Homeward Bound Demonstration and Money Follows the Person program, including transitions from nursing facilities into targeted waiver programs and ensure consistent application of transition tools, procedures, and workflows as outlined in the Operational Protocol.
  • Manage MFP grant-funded activities and contracts, including budget development and monitoring, invoice review, deliverable tracking, and required state and federal reporting.
  • Serve as the primary liaison with the Centers for Medicare and Medicaid Services (CMS) for MFP-related activities, including participation in technical assistance calls, coordination of site reviews, and response to federal reporting and compliance requirements.
  • Lead data collection, analysis, and quality improvement activities related to adult LTSS HCBS transition outcomes, service delivery, and program effectiveness, including supervision of staff supporting quality management and reporting functions.
  • Collaborate closely with the Office of MaineCare Services, other DHHS programs, and external partners to resolve complex transition barriers, address system-level issues, and ensure continuity of Medicaid services.
  • Develop and deliver training and technical assistance to internal staff, external stakeholders, provider agencies, facilities, and partner organizations to support effective, person-centered transitions across the LTSS HCBS system of care.
  • Manage and maintain Microsoft 365 and SharePoint-based systems used to support MFP and LTSS HCBS program operations, tracking, reporting, and cross-agency communication.
