We are currently seeking a compassionate and reliable Transitional Aide to join our team. Requirements
- MUST have FULL flexible availability (days, evenings, weekends as needed)
- Reliable transportation
- Ability to assist clients during transitional care periods
- Strong communication and time-management skills
- Dependable and professional demeanor
Responsibilities
- Provide support and assistance to clients during care transitions
- Assist with daily living activities as needed
- Follow care plans and agency policies
- Communicate effectively with supervisors and care team
