About The Position

As a Transitional Housing Youth Advocate, you will provide case management services for participants in Story, Green, Hardin, Marshall, and Boone Counties. You will provide intensive one-on-one services to homeless or potentially homeless individuals who are residing in scattered-site houses, and apartments. Other duties include providing crisis intervention for runaways, following up with the youth and families, referring individuals for services, assisting to facilitate placement, and providing screening of runaway and homeless youth. A day in the life of a Transitional Housing Youth Advocate can look like: Provide assessments, intake, and discharges on approved participants. Assess housing barriers and determine service needs with the overarching goal of self-sufficiency and housing stability. Relate to all participants in a caring, supportive and non-judgmental manner. Ensure a safe living environment for all participants. Use trauma-informed care techniques when working with participants. Provide crisis intervention and mediation when required in situations that demand immediate attention. Work with participants to set both short-term and long-term goals, and evaluate progress on goals. Make appropriate referrals to community resources and assist in scheduling appointments and provides transportation, as necessary. Meet with all participants individually at least weekly and check in on participants regularly. Plan and conducts a life skills group, teaching youth skills around independent living (i.e. thermostat, setting up utilities, grocery shopping, cooking, use of kitchen appliances, cleaning best practices, what does and does not go in a toilet, setting up mail service, etc.) Assist in locating and furnishing homes and apartments. Complete case plans, progress notes, assessments, and other required paperwork and data entry in a timely fashion. Attend required meetings and conference calls. Other duties as assigned.

Requirements

  • Bachelor's degree in social work or related human services field.
  • Two (2) or more years of related experience.
  • At least 21 years old with a valid driver's license to meet requirements to drive agency vehicles.
