The Transplant Liaison Team plays a vital role in guiding End-Stage Kidney Disease (ESKD), and waitlisted patients through the transplant process. Our Transplant Liaison Team acts as a key advocate, ensures seamless support from referral to transplant completion, working closely with the market Transplant Provider partners, patients, care partners, and healthcare teams to facilitate access to transplantation. Serving as a bridge between healthcare providers, transplant centers, and the Integrated Kidney Care (IKC) team, the Transplant Liaison Team supports transplant evaluations, optimizes wait listing processes, and promotes living donor awareness. By addressing barriers to transplant, the Transplant Liaison Team helps mitigate delays and drives optimization of the transplant process. The Transplant Liaison Team LEAD provides supervision and support to our Transplant Liaisons. Keep reading to see if this could be the right position for you! NOTE: Expected to regularly travel to meet with teammates in designated region and surrounding areas 1-2 days per week, working the remaining days from home. Team Leadership & Daily Operations Provide day-to-day guidance under the supervisor's direction. Organize, monitor, evaluate, and support TL workflow for timely and consistent execution. Demonstrate the ability to lead a team, delegate tasks, and provide clear work direction. Drive program outcomes and key performance metrics by supporting TLs in overcoming barriers and optimizing patient progression Facilitate ongoing teammate development through: coaching, mentoring, and continuous training. Attend and/or lead team meetings, trainings, operational calls, and partner sessions as required. Demonstrate consistent leadership qualities, professionalism, and reliability Maintain strict confidentiality at all times when handling patient, teammate, and organizational information.
