About The Position

The Transportation Coordinator at Health Systems Management (HSM) supports and works under the direction of the Social Work Manager and the Clinical Nurse Manager as part of the hemodialysis Healthcare Team. Delivering safe and high-quality transportation services to our dialysis patients. The Transportation Coordinator will provide optimal transportation education/options to all dialysis patients, care givers, and/or family members through a team approach. Adhering to all requirements of the HSM Compliance Program, including patient care and administrative policies.

Requirements

  • High School Diploma or Equivalent.
  • Valid State Driver’s License and reliable transportation.
  • Ability to read, analyze and interpret general business manuals, and governmental regulations.
  • Demonstrated proficiency with Computers.
  • Proficiency with all aspects of Microsoft Office (Excel, Word, Outlook, Spreadsheets, etc.) writing reports and correspondence.
  • Ability to present information and respond to questions from groups, manager, staff, clients, patients, etc.
  • Ability to demonstrate critical thinking skills, sound judgement, and problem-solving skills.
  • Customer focused attitude, with high level of professionalism, confidentiality, and discretion.
  • Excellent verbal and written communication skills.
  • Ability to be flexible with schedule.
  • Employee must successfully pass the Ishihara’s Color-Blind Test.

Nice To Haves

  • Associate degree from an accredited program.
  • Previous experience with ESRD patients.
  • Experience working in a medical setting and/or transportation field.

Responsibilities

  • Handle high volume of calls regarding Patient transportation needs/issues from each HSM Georgia clinic.
  • Handle crisis/issues/concerns throughout the day regarding transportation.
  • Educate Patient’s and family members about transportation options.
  • Assist with scheduling transportation for new or existing Patients that are eligible for these services.
  • Obtaining and faxing required paperwork/medical records required for transportation services.
  • Creating, updating, and maintaining the transportation logs/booklets for each Georgia clinic.
  • Communicate with Social Workers, Managers, Transportation Providers, Floor Staff and Doctors regarding transportation services.
  • Completing and maintaining monthly reports.
  • Ability to work independently, multi-task, plan and organize priorities autonomously.
  • Travel between dialysis clinics (as needed).
  • Perform additional duties as assigned.
