Requirements

Current CNA certification and OBRA registry
Ability to pass MVA Background Check
Excellent interpersonal skills
Fast and adaptive problem-solving abilities
In-depth understanding of patient care policies and regulations
First aid and CPR certifications

Responsibilities

Transport and escort residents to/from medical appointments, activities, outings, and general errands
Ability to organize scheduled appointments throughout the day
Drive a wheelchair lift van, which is specially equipped with an automatic lift
Responsible for obtaining directions to the location before leaving facility
Communicate with facility about any pertinent information related to resident transportation
Monitor and report any changes in condition with assigned residents
Complete all transports to/from appointments with promptness, communicating all late arrivals/departures to clinic/facility
Assist on the floor answering call lights as a NAC when there is downtime
Collaborate with other team members to ensure quality of care
Work with appointment scheduler on scheduling appts and transport
