About The Position

Position Purpose: Mercy Care Atlanta is seeking a compassionate and skilled Access Center LPN to join our dedicated team. This role provides telehealth support including patient assessment, education, crisis intervention, counseling, and triage via phone or other interactive media. The RN will support all clinical departments and occasionally assist in clinic operations, promoting health and wellness in underserved communities. What You Will Do: Conduct telephonic assessments and provide education, counseling, and triage using established care protocols. Support clinic operations as needed, performing health screenings (blood pressure, glucose, vision, TB, etc.) and administering flu shots. Obtain medical histories and evaluate overall health status. Collaborate with Health Educators to deliver one-on-one education on chronic conditions such as diabetes, hypertension, and anemia. Make referrals to Mercy Care and other providers, and participate in care planning as part of an interdisciplinary team. Provide health coaching and support for chronic condition management, including goal setting and skill building. Participate in community health fairs and events 3–4 times per year. Contribute to office-based population health initiatives through workflow redesign and staff training. Deliver age-appropriate care for adults (18–65), geriatric patients (65+), and occasionally infants, children, and adolescents.

Requirements

  • Associate degree in Nursing required.
  • Active Georgia LPN license.
  • CPR certification.
  • Minimum two years of experience, preferably in outpatient or telephonic nursing.
  • Strong desire to serve underserved and disadvantaged populations.
  • Excellent communication and patient education skills.
  • Knowledge of chronic disease management, adult and pediatric health, and low-literacy patient education.
