About The Position

Triage Registered NurseWhat You Must Have: Current State Registered Nurse License Current State Driver’s License, reliable transportation, and provide ongoing valid and current auto liability insurance Two years of experience as an RN preferred in a healthcare/clinical setting Full Time Employee Benefits Competitive Pay Medical, Dental & Vision insurance Paid Time Off Paid holidays 401k with up to 4% employer matchingTuition reimbursement Company car for qualifying individuals Mileage reimbursement What You Will Do Provide telephonic triage to hospice patients and their families during after- hours, weekends, or at other times when office is closed or at the request of the Executive Director Ensure after-hour visits are scheduled with the on-call team in a timely manner Ensure documentation is completed and submitted to the site at the end of each shift to ensure continuity of care for the week day team Manage referrals that come in over the weekend and after-hours Provide education and support to patients and families/caregivers Conduct patient visits as needed (admissions, routine, pronouncements, tuck-ins) Promote positive, respectful communication to all staff, referral sources, patients and families Performance of other duties as required Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures

Requirements
