Position Summary The Vocational Transition Specialist focuses on building person-centered, recovery oriented services, vocational skills, employment readiness, and community integration through comprehensive assessments and the development and implementation of individualized vocational service plans. The Specialist supports individuals in progressing toward employment, education, training, and independent living while promoting long-term stability and recovery.

Requirements

  • The Vocational Transition Specialist must successfully complete and demonstrate proficiency in agency provided training, including: CPR and First Aid Crisis intervention and de-escalation Trauma informed care Harm reduction principles Motivational interviewing and stages of change Substance use awareness and relapse prevention Recovery oriented and strengths-based practices
  • Bachelor’s degree with a minimum of two (2) years of experience in mental health services, with a focus on vocational rehabilitation, employment readiness, or job placement preferred OR Associate’s degree with three (3) or more years of relevant experience in mental health, vocational services, workforce development, or related fields
  • Valid New York State Driver’s License

Responsibilities

  • Vocational Services & Skill Development Provide direct vocational and rehabilitative services to assigned individuals, including planning, implementation, and monitoring of individualized goals. Services include, but are not limited to: Employment readiness and career exploration Vocational assessments and job preparation Resume development, interview preparation, and workplace skills Budgeting and financial literacy Activities of daily living (ADLs) and independent living skills Grooming, hygiene, and workplace presentation Nutritional planning and basic meal preparation Travel training and use of public transportation Community integration and engagement in community resources Shopping, menu planning, and household management Social, interpersonal, and professional communication skills Assertiveness and self-advocacy training Support with transitions related to employment, education, housing, or treatment settings
  • Assessment & Vocational Service Planning Conduct comprehensive vocational and functional assessments Develop, implement, and regularly update individualized vocational service plans Collaborate with individuals to establish realistic, meaningful short and long term goals Monitor progress toward vocational and transitional goals and adjust services as needed
  • Coordination, Collaboration & Advocacy Coordinate services with internal WellLife Network programs and interdisciplinary teams Establish and maintain collaborative relationships with employers, training programs, educational institutions, and community partners Facilitate referrals and linkages that support vocational development and transitions Advocate for individuals to access opportunities, accommodations, and community resources
  • Documentation & Administrative Responsibilities Maintain accurate, timely, and organized case notes and service documentation Track service delivery, goal progress, and outcomes Participate in team meetings, case conferences, and supervision Assist with onboarding and follow up for individuals transitioning into or out of programs
  • Community-Based & Transitional Support Support individuals in planning and participating in vocational, recreational, and social activities Escort individuals to interviews, appointments, trainings, or community activities when needed Support safe participation in community based services and activities
  • Provide coverage and support during staff absences as needed
  • Represent WellLife Network at meetings, trainings, and community events
  • Maintain flexibility in schedule and travel between program locations
  • Transport self between sites and operate agency vehicle when required
  • Maintain confidentiality in accordance with agency policy
  • Perform other duties as assigned by supervisor
