Job Description Weekend Admission Coordinator Schedule: Saturday and Sunday 8a-7p Demonstrate commitment to the Paradigm Health mission, vision and core values through your daily job performance. Provide leadership and guidance to intake team members, and admissions nurses, including training, mentoring, and performance evaluation. Foster a positive and collaborative work environment within the intake team and admission team, promoting teamwork and professional growth. Act as a liaison between team members and management, communicating feedback, concerns, and suggestions for improvement. Participate in meetings, trainings, and committees as necessary to support organizational goals and initiatives. Responsible for establishing and maintaining positive relationships with customers, both internal and external, as well referral sources within the assigned settings (hospital, facility, home, physician offices, etc.). Support the business development team and other intake team members in obtaining required information & documentation for incoming referrals. Assess clinicals for patient eligibility and ensure required documentation is obtained as needed to support eligibility, proper coding and accurate billing in accordance with home health Medicare regulations. Review medications/treatments and assess that patient/POA goals of care align with hospice philosophy. Assist clinical liaisons and admission nurses with assessment of patient safety in home environment. Benefits Competitive salary Excellent PTO program Every Friday is payday! Great team environment! Requirements Medical Office experience is required Nursing License in Indiana Hospice Intake Experience Strongly Preferred Proficient in Microsoft excel Flexible, organized, and exercises sound judgment. Requirements

Medical Office experience is required
Nursing License in Indiana
Proficient in Microsoft excel
Flexible, organized, and exercises sound judgment.
Meticulous attention to detail, chart accuracy, and compliance.
Excellent time-management.
Strong communication (written and verbal) skills
