About This Role: Care Coordination & Patient Engagement: Accepts warm hand-offs from medical and behavioral health providers to ensure seamless integration of care; engages patients (adults and children) and families in setting and achieving self-care goals through individualized care planning and support. Health Education & Coaching: Delivers in-person and virtual health education sessions—both individual and group—using evidence-based curricula to support patients’ self-management and wellness. Case Management & Support Services: Provides case management, tracks patient progress, coordinates referrals to community resources, and monitors ongoing care plans to ensure medical, behavioral, and psychosocial needs are met. Scheduling & Access to Services: Coordinates and schedules appointments across multiple service lines, including medical, dental, nutrition, and behavioral health; educates patients on available Evara Health services and facilitates access. Reporting & Committee Participation: Collects and reports program data, presents patient case reviews, participates in CMHI Oversight Committee, and supports special projects and organizational compliance with HIPAA, HRSA, and other regulatory standards. Requirements

  • Minimum of three years’ experience with minimum qualifications listed above

Nice To Haves

  • Bachelor’s Degree in Social Work, Health or Human Services field, preferred
