About The Position

As a Group Home Case Manager you will be a member of a care-team responsible for leading and maintaining a safe, supervised, and structured living environment for adolescent residents that is conducive to their well-being. Case Managers assess group dynamics and resident needs to then utilize that information when carrying out the essential duties and responsibilities of this position. Case Managers promote improved social, developmental, educational and physical functioning of group home residents. Successful Group Home Case Manager candidates possess a high attention to detail with great time management and organization skills. This position requires effective communication skills as well as the ability to work efficiently in a fast-paced environment. Requirements

  • Bachelor’s degree in a human/social services related field preferred
  • Previous experience working with a youth population in a residential setting
  • Excellent interpersonal and communication skills
  • Ability to build relationships with residents, team members, and other treatment team professionals
  • Action-oriented and ability to work in a fast-paced environment
  • Previous case management experience
  • At least 21 years of age
  • Hold a valid driver’s license

Responsibilities

  • Ensure programming adheres to state licensing requirements by supporting program policy and procedures
  • Accurately complete resident file documentation and incident reports and completing documentation and incident reports within the allotted timeframe.
  • Serve as program contact for families, social workers, law enforcement, and other collateral contacts.
  • Participate in resident review meetings.
  • Follow resident treatment plans and communicate treatment goals with group home staff.
  • Incorporate resident treatment goals into the individual care of the residents.
  • Schedule resident appointments such as therapy, counseling, medical, dental, or other external resources and coordinate transportation.
  • Coordinate resident education needs.
  • Accompany residents to scheduled proceedings.
  • Coordinate client home visits in accordance with treatment objectives, phase, level, and program guidelines.
  • Monitor and report resident progress to family, courts, social workers, or other collateral contacts.
  • Conduct client intakes and discharges.
  • Process and communicate resident medication changes.
  • Referral screening with oversight from the Group Home Manager/Director.
  • On-call responsibilities as assigned.
  • Maintaining a safe, supervised, and structured living environment for our adolescent clients
  • Supervising youth and knowing their whereabouts at all times
  • Facilitating crisis counseling, conflict resolution, and problem solving with and for clients
  • Incorporating client treatment objectives into the individual care of residents
  • Transporting and supervising clients at designated activities and appointments
  • Accurately completing and reviewing shift documentation
  • Training new hires following the defined training program
  • Additional duties as assigned
