Youth Transition Specialist
About The Position
We are seeking a Youth Transition Specialist to join our team. Starting Salary: $46,000 - $50,000 (Salary) This role will be a coach and mentor for young adults, guiding them into employment and educational opportunities. This position is in our signature Flourish program within our Build Trybe department. Flourish is dedicated to helping young adults find and sustain success. The Youth Transition Specialists will begin working with youth midway through their Build Trybe pre-apprenticeship and carry an expected caseload of 50 youth each. This position works with other team members and reports to the Manager of Flourish
Requirements
- Our ideal candidate will have 3 years of experience in college & career readiness planning or guidance related field
- Bachelor’s degree, required
- At least 21 years of age and pass background check, physical, and drug screening
- A valid driver’s license in the state you reside in, proof of current vehicle insurance, and reliable transportation.
Responsibilities
- Develops transition plan models, designs and implements plans, and assesses student progress.
- Disseminates information to staff, students, and parents regarding student needs, progress, and resources.
- Coordinates transition activities for college, career, and community readiness.
- Facilitates referral, application, and eligibility process for students and families to adult service providers.
- Identify & refer individual to correct Career Pathway Program for placement.
- Conduct periodic review & progress monitoring of youth in caseload.
- Provide workshops and classes.
- Connect young adults to resources that lead to employment and educational success.
