As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care. Please note the position requires you to meet patients in their homes throughout Duval County, FL and you will be expected to cover a territory radius of 50 minutes from your home address.

Requirements

  • Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
  • Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
  • Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
  • MVR with reliable transportation and clean driving record.
  • DEA license and BLS certification
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.
  • Frequent use of computer and phone systems
  • Must be able to constantly remain in a stationary, sitting position, communicate and exchange information with others, inspect information, perform repetitive motions with arms and fingers, interpret data, problem solve, make decisions, organize and plan, and maintain a positive and professional attitude in all situations
  • Ability to lift and carry up to 20 lb
  • Work is performed in patient homes or facilities and may involve exposure to varied conditions.
  • Regular travel to patient homes, provider facilities, or community locations. Employees may be exposed to varying weather conditions, road hazards, and unfamiliar environments.

Responsibilities

  • Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
  • Manage chronic conditions and develop individualized treatment plans.
  • Serve as the primary care provider and conduct follow-ups.
  • Maintain accurate EMR documentation and comply with healthcare regulations.
  • Educate patients on disease management and healthy living.
  • Collaborate with interdisciplinary teams to ensure coordinated care.
