As a Provider at Lucet, you’ll deliver comprehensive care to patients in their homes and/or community-based services such as assisted living or skilled nursing facilities, helping to improve outcomes and promote continuity of care. Please note the position requires you to meet patients in their homes throughout Maricopa County, AZ and you will be expected to cover a territory radius of 50 minutes from your home address.

Requirements

  • Active, unrestricted current licensure as a Provider in the state in which you will be providing care for patients. License must be clear of any clinical and/or board practice restrictions.
  • Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant
  • Obtain autonomous provider status as required by state you are licensed with currently within six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
  • Ability to pass background check upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/trace search (LEIE), education report (highest degree obtained), civil upper and lower search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
  • MVR with reliable transportation and clean driving record.
  • DEA license and BLS certification
  • Arizona market specific: 1+ year of experience in primary care or a related field such as family practice, general practice, or outpatient care.
  • High-speed internet service (cable or fiber optic) with minimum download Speed of 20 Mbps, Upload Speed of 5 Mbps, and Maximum Latency of 100 milliseconds (must be installed before starting) required.

Responsibilities

  • Clinical Care & Patient Management:
  • Conduct in-home visits (minimum 25/week), including assessments, exams, and medication reviews.
  • Manage chronic conditions and develop individualized treatment plans.
  • Serve as the primary care provider and conduct follow-ups.
  • Maintain accurate EMR documentation and comply with healthcare regulations.
  • Patient Engagement & Collaborative Care
  • Educate patients on disease management and healthy living.
  • Collaborate with interdisciplinary teams to ensure coordinated care.

Benefits

  • Annual compensation between $120,000-$143,000, PLUS an annual performance-based, discretionary incentive.
