Acclaim Multispecialty Group, a partner with JPS Health Network is seeking a motivated and ambitious advanced practice provider to join our dynamic Street Medicine Team. As the county hospital system, JPS has been serving the residents of Tarrant County for over 100 years. Job Summary: The Street Medicine Medical Director provides strategic, clinical, academic, and operational leadership for the Acclaim Street Medicine program within JPS Health Network. This role oversees the delivery of healthcare to individuals experiencing unsheltered homelessness across community settings, while advancing JPS’s safety-net mission through clinical excellence, interdisciplinary collaboration, education, and community partnership. Street Medicine operates at the intersection of healthcare, public health, and social systems. The Medical Director ensures that services provided in non-traditional environments maintain high clinical standards, strong institutional integration, and meaningful impact for some of the community’s most medically and socially vulnerable populations. The position serves as a critical bridge between the healthcare system and the community, supporting continuity of care for patients who often face significant barriers to traditional medical services.

Requirements

  • Must be comfortable working in a large public/county facility
  • Must be able to demonstrate a working knowledge in area assigned, and competency in assessment techniques, diagnosis and treatment processes that apply to the assigned medical specialty area
  • Must have good interpersonal, verbal and oral communication skills to work effectively with other health care professionals and serve as a resource to the community
  • Must be self-directed and take initiative to develop and participate in community activities and medical outreach programs pertaining to the medical specialty area
  • Master of Science in Nursing (MSN) from an accredited an accredited Nurse Practitioner program OR Graduate of an accredited Physician Assistant Program with a Master’s degree.
  • Current, unrestricted Texas Advanced Practice Registered Nurse with Acute Care Nurse Practitioner (ACNP) certifications. OR Current, unrestricted Texas Physician Assistant license.
  • Unrestricted work authorization to work in the United States.

Responsibilities

  • Academic and Educational Leadership
  • Potential to serve as Assistant Program Director for the JPS Family Medicine Residency
  • Co-directing the JPS Street Medicine Fellowship program with the IES Emergency Medicine faculty which is the funder and owner of the fellowship program
  • Teaching and supervising residents, fellows, medical students, and interdisciplinary trainees
  • Developing curriculum related to Street Medicine, trauma-informed care, and care for underserved populations
  • Mentoring learners in scholarly activity, quality improvement projects, and research initiatives
  • Representing JPS Street Medicine in academic forums, conferences, and collaborative networks
  • Overseeing and ensuring high quality monthly didactics for the Street Medicine track residents and fellow
  • Overseeing the running of an annual Street Medicine Boot Camp to train the incoming longitudinal learners
  • Community Engagement and Partnership Development
  • Maintains regular relationships with shelters, outreach teams, housing providers, behavioral health agencies, and municipal partners at high levels through regular correspondence, attending meetings, etc.
  • Facilitates coordinated care across community systems
  • Participates in case conferencing and collaborative problem-solving for complex patients
  • Represents JPS Health Network within community initiatives addressing homelessness and health disparities
  • Supports system-level improvements that strengthen the community safety net
  • Key point of medical care input for people experiencing homelessness across the local Continuum of Care
  • Key point of input of how to conduct street-level outreach to people experiencing homelessness for the local Continuum of Care
  • Program Leadership and Advocacy
  • Ensuring the Street Medicine program maintains its mission-driven focus on dignity-centered care for Tarrant County’s unsheltered unhoused population
  • Communicating program impact through data reporting, institutional presentations, and community engagement
  • Identifying systemic barriers affecting unsheltered patients and advocating for improvements in care access
  • Contributing clinical and public health expertise to discussions regarding homelessness and healthcare delivery
  • Routinely involved with public relations in accordance with and knowledge of the JPS Communications Department.
