The Behavioral Health Case Manager III (BHCMIII) position sits in the Child and Family Care Coordination Unit which works with children and adolescents with serious behavioral health conditions and their families to help families access treatment and other services/resources that help the child/adolescent achieve success at home, school and in their community. The BHCMIII works directly with providers to monitor progress in treatment and works with community partners including child welfare, juvenile justice, schools and the Courts to help families navigate those systems. The BHCMIII works with a licensed clinician to support new staff through shadowing and other opportunities and also completes chart reviews for team members to ensure alignment with unit standards. An employee in this class may participate as a member of a multi-disciplinary team to implement behavioral health services or treatment plans in accordance with Delaware Code, Title 24.
