Requirements

  • Master’s degree in social work with limited or full licensure, or Licensed Professional Counselor licensure required
  • BLS certification required or ability to obtain
  • Strong understanding of behavioral medicine and evidence-based treatment approaches
  • Comfortable working closely with primary care providers in a team-based setting
  • Strong clinical assessment, communication, and documentation skills
  • Excellent organizational, customer service, and time management skills
  • Ability to maintain strict patient confidentiality

Nice To Haves

  • Minimum of 2 years of Social Work experience preferred

Responsibilities

  • Provide individual, family, and group counseling, as well as crisis intervention
  • Complete and document mental health screenings and assessments
  • Develop and maintain treatment plans with patient participation
  • Deliver clinically appropriate behavioral health treatment and support
  • Assist primary care providers in identifying and addressing mental health and psychosocial concerns
  • Support patients with behavior change, self-management skills, and lifestyle goals
  • Identify barriers to treatment plan follow-through and help connect patients to additional services
  • Lead patient education and support groups
  • Collaborate with care team members to coordinate patient care
  • Maintain timely and accurate documentation
  • Participate in peer review, case consultation meetings, and quality improvement initiatives
