Facilitates, coordinates, and organizes behavioral health care in a team-based environment, to meet patient needs and achieve optimal health outcomes for adult and pediatric patients. Provides care management services of mental or behavioral health conditions, addresses social and psychosocial factors, integrating behavioral health with medical care in support of the primary care practitioner. Identifies and addresses social needs preventing optimal health. Connects patients with needed community resources and services to improve health outcomes. Documents care and services in the electronic medical record as a member of the care team. Utilizes policies that support patient's fundamental right to considerate care, protects their personal dignity and respects their cultural, psychosocial and spiritual values in an effort to meet patient care needs and preferences. Reports to the ambulatory care management leader. Performs other duties as assigned.
