POSITION SUMMARY Under the supervision of the Program Director, CASAC is responsible for facilitating individual and group substance use therapy as well as completing intakes, assessments, session notes, treatment plans and discharges. The CASAC will be an integral part of the interdisciplinary team servicing individuals at the Success Community Care Clinic. CASAC provides crisis intervention, direct support, and oversight, assistance with daily living skills, and any other assistance that individuals need while participating in site-based services. The team member will assist in the development of the Initial Treatment Plans, Relapse Prevention Plans, and Support Plans. The CASAC will also run therapy groups on specific topics, provide individualized supportive counseling, attend interdisciplinary team meetings, and conduct assessments as needed or required. DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS Facilitates individual and group sessions for substance use, mental health, co-occurring disorders, medication management, and other outlying subjects related to this population.Works with clients to improve life choices and maximize benefits of program. Assists social workers build, coordinate, and lead programs supporting individuals with substance use and co-occurring disorders.Provides crisis intervention services for family members and individuals served.Provides family counseling and education.Provides support and implements 12 step programs, self-help recovery services, harm reduction, motivational interviewing and other models and approaches as needed.Displays sensitivity to cultural and linguistic needs of clients and family served.Participates in various multidisciplinary team meetings as needed. Attends weekly supervision with Clinic Director and follows through on any directives.Completes notes, treatment plans and discharge planning in a timely manner.Conducts intakes, admission screenings, and assessments for referred clients.Participates in comprehensive service planning for each assigned client in collaboration with other agencies.Collaboratively develops individual recovery plans and monthly updates for each individual.Enforces policies, procedures, rules, and regulations consistently in alignment with service delivery and client care.Identifies and assesses clients at risk and provides crisis intervention for individuals as needed.Supervises toxicology’s as needed. Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
