About The Position

The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Housing Locator & Mental Health Professional (MHP) Street Outreach who demonstrates this mission and wants to work for an organization that makes a difference in the community. Schedule: Monday-Friday 9am-5m (Flexible) What will you be doing? The Case Manager MHP must complete documentation of mental health services provided in accordance with state and agency standards as applicable. The Case Manager MHP is responsible for assisting clients in locating and maintaining housing and assisting with barriers to living in the community to live successfully in permanent supportive housing. The Case Manager is expected to maintain professional and effective relationships with agency clients and landlords. The Street Outreach Case Manager is responsible for contacting individuals who are inappropriately using Crisis Services through Police and Fire, identified by FUSE committees, or located in tent cities and other public spaces and attempting to engage them in more clinically appropriate services and access shelter/housing. The case manager will provide outreach and case management, advocacy, evidence based mental health and support services to individuals with mental health diagnosis and provide appropriate and warm hand offs to referrals. Case Managers will engage clients in the community where they are located and will assist in client’s identifying natural supports. Hours include evenings and weekends. Case managers will need to complete all AID mandatory trainings and any additional trainings that are assigned.

Requirements

  • Education: Bachelor's degree in human services field required
  • Experience: Experience with locating housing or working in housing related field.
  • Knowledge of tenant and landlord rights/laws
  • Experience with at-risk clientele and crisis work.
  • At least 1 year of experience in behavioral health and case management preferred
  • Valid Illinois driver’s license
  • Drive self and clients in personal vehicle
  • Must have the minimal amount of liability insurance on personal automobile as defined by AID handbook

  • Masters Degree preferred
  • License or Certification: ASSIST and SAMHSA SOAR certification preferred.
  • Bilingual preferred

Responsibilities

  • Meet minimum service hour provision productivity standards (MRO) monthly.
  • Perform outreach and home visits.
  • Complete all required case management documentation consents, residential/agency paperwork) on a timely basis.
  • Complete and sign all MRO Documentation within 48 hours using Cx360
  • Meet with all assigned clients on regular basis depending on program and client needs; submit daily activity logs.
  • Assist clients with activities of daily living, including apartment maintenance, and provide client training consistent with the treatment goals.
  • Provide guidance to MHP and RSA staff regarding implementation of IM+CANS goals/objectives.
  • Obtain Input from clients, families, guardians and other staff on how to improve residential services.
  • Monitor and report safety of clients’ living arrangement
  • Acquire and maintain required trainings and certifications as well as any other trainings assigned by Manager.
  • Assist clients in paying bills, budgeting and banking per program procedure.
  • Oversight or management of client funds when necessary (dual signature accounts, AID rep. Payees).
  • Obtain and maintain client benefits (Social Security, Medicaid, Link Card, Etc.).
  • Assure client records are properly maintained per agency procedures.
  • Complete authorizations, reauthorizations and spend-down paperwork in a timely fashion.
  • Update Cx360 with corrections whenever necessary, but at least annually to ensure accuracy of records.
  • Provide effective services for clients’ individual needs and in line with client rights and the Mental Health Recovery Model.
  • Attend monthly team meetings and be a positive contributor.
  • Recognize emergency situations and take appropriate action.
  • Contact Manager and Director per procedure.
  • Complete necessary paperwork correctly (incident reports, petitions, encounters).
  • Coordinate and provide linkage and referrals to other community support services.
  • Knowledge, understanding, and implementation of principals of housing first, and harm reduction.
  • Ability to work with diverse population, with specific concentration in mental health, substance abuse, intellectual developmental disabilities, and physical disabilities.
  • Ability to develop relationships with community landlords in order to quickly find clients community housing.
  • Coordinate and provide care that is safe, timely, effective, equitable, and client centered.
  • Attend to scheduled work hours and be flexible to meet client and program needs, as assigned by Program Manager or Director.
  • Respond to outreach calls from shift start to shift end.
  • Respond to calls upon being dispatched
  • Attend soup kitchens, breakfasts and community dinners
  • Provide advocacy and support to individuals experiencing mental health concerns
  • Complete clinical assessments that meet the requirements of program funders and applicable administrative rules
  • Provide case management services as medically necessary
  • Complete required documentation (assessments, treatment plans, case notes, VISPDATs, consents, monthly case checks, ROI) on a timely basis
  • Provide appropriate follow-ups to establish rapport with individual.
  • Acquire/maintain certification in First Aid, CPR, CPI, Mandated Reporting, and other agency required trainings.
  • Maintain professional judgment and performance regardless of personal issues
  • Contact people inappropriately using Crisis Services and attempt to engage them in more clinically appropriate services.
  • Complete and maintain monthly and quarterly assigned reports.
  • Maintain and update Outlook calendar with daily details of activities.
  • Input information into HMIS.
  • Promote a relationship between clients’ families and additional supports.
  • Provide transportation to clients in personal or agency vehicles when necessary for work, shopping, appointments, etc.
  • Maintain a positive and professional working relationship with coworkers, agencies and other stakeholders.
  • Maintain a team approach to services.
  • Maintain client rights.
  • Assure the coordination of services in the community to meet client needs (i.e. Financial, legal, advocacy, medical, ect.)
  • Promote community / family involvement in facility programs.
  • Participate in and promote organizational and division fund-raising and annual events.
  • Advocate on behalf of clients and make recommendations as needed.
  • Attend staffing with other community agencies as needed
  • Implement Coordinated Entry per System Coordination and Entry Policies and Procedures
  • Learn and utilize SOAR when applying clients for benefits
  • Ensure accurate data quality standards in HMIS – No less than 90% accuracy for the program
  • Acquire and utilize HMIS service center, data base, word processing software
  • Learn and utilize HUD/ESG
  • Assist clients in moving into or out of housing units
  • Assist clients with housing needs (e.g., Transitioning in and out of apartment program, developing skills necessary to live independently)
