About The Position

Full Time Adult Outpatient Monday – Friday between 8am and 6pm $22.06 - $30.10 Pierce County's first Certified Community Behavioral Health Clinic Scope Provide case management services as a member of the Outpatient team who is working with clients in a clinic and community setting. Provide counseling and case management services to include individual or group sessions to clients. Work may be done in the outpatient clinic and in the community. Work can include driving as needed for or with client for case management needs as well as navigating resources with or on behalf of client. Serves as a member of the organization that welcomes, engages and serves individuals with complex needs

Requirements

  • AA degree in a human services field with two years relevant experience or BA required.
  • WA State credential or ability to become credentialed
  • If this role requires driving, must have valid driver’s license, proof of insurance, and working vehicle.
  • Proficiency in common Microsoft applications
  • Strong written and verbal communication skills
  • Demonstrated ability to prioritize deadlines, work independently, take initiative, and maintain confidentiality
  • Must have the ability to: Sit for 2 total hours per day, and up to 1 total consecutive hour Stand for 8 total hours per day and up to 2 total consecutive hours Frequently use fingers/hands for work that requires dexterity Use upper extremities for frequent lifting of objects up to 10 pounds Ability to transport clients in personal vehicles.
  • Must have the ability for: Frequent stooping and bending Touching as it relates to feeling objects by touching with the skin, particularly that of fingertips Hearing as it relates to normal conversation, high frequency and low frequency hearing sounds Seeing as it relates to general vision, near vision, color vision and peripheral vision

Responsibilities

  • Provide both case management and behavioral health support services, including, but not limited to assisting clients with paperwork, self-care, advocacy, and resource acquisition.
  • Be trained and use Motivational Interviewing and other evidence-based practices
  • Provide psychoeducation to clients and their natural supports about behavioral health conditions, treatments and navigating systems.
  • Provide case coordination, working within a multi-disciplinary team environment.
  • Facilitate psychoeducation and skills groups as defined by program needs.
  • Connect clients with internal and external supports and services.
  • Attend/facilitate treatment team meetings, staff meetings, and designated staff training and consultation sessions as assigned.
  • Ensure that ethical and professional standards of practice are maintained by following all CLR policies and seeking regular consultation.
  • Actively participate in clinical supervision and training for continued professional growth.
