The Lazarus Program is a specialized outreach initiative focused on engaging individuals who are chronically homeless in Miami-Dade County. Through street outreach, comprehensive assessment, and coordinated care, the program supports clients in accessing medical and behavioral health services and securing permanent supportive housing. What You Will Do As a Case Manager I within the Lazarus Program, you will serve as the primary point of contact for clients, leading outreach, engagement, and care coordination efforts from initial contact through housing placement. The majority of your work will take place in the field, engaging individuals experiencing homelessness and supporting their transition into stable housing. You will collaborate with internal teams and community partners to develop individualized service plans, remove barriers, and ensure clients receive the support needed to achieve long-term stability.
