The Lazarus Program is a specialized outreach initiative focused on engaging individuals who are chronically homeless in Miami-Dade County. Through street outreach, comprehensive assessment, and coordinated care, the program supports clients in accessing medical and behavioral health services and securing permanent supportive housing. You bring strong interpersonal and communication skills, cultural sensitivity, and the ability to build trust with individuals who may have experienced trauma, severe mental illness, and substance use disorders. What You Will Do As a Case Manager I within the Lazarus Program, you will serve as the primary point of contact for clients, leading outreach, engagement, and care coordination efforts from initial contact through housing placement. The majority of your work will take place in the field, engaging individuals experiencing homelessness and supporting their transition into stable housing. You will collaborate with internal teams and community partners to develop individualized service plans, remove barriers, and ensure clients receive the support needed to achieve long-term stability.

Requirements

  • Education: Bachelor’s Degree in Social Work, Mental Health, or a related field required.
  • Licensure: Valid Florida Driver’s License with a clean driving record required.
  • Screening Requirements: Successful completion of criminal background check, toxicology screening, and Level II screening.
  • Language Skills: Proficiency in spoken and written English required; Spanish preferred; Creole or additional languages a plus.
  • Technical Skills: Computer literacy required, including Microsoft Office and data entry systems.
  • Experience: Prior experience working with individuals experiencing homelessness or vulnerable populations preferred.
  • Professional Skills: Strong organizational, interpersonal, and communication skills with the ability to manage multiple priorities and meet deadlines.
  • Personal Attributes: Self-motivated, adaptable, detail-oriented, and able to remain calm and effective under pressure.
  • Cultural Sensitivity: Ability to work respectfully with diverse populations, including individuals with histories of trauma, mental illness, and substance use.
  • Commitment: Strong alignment with Housing First principles and belief in each individual’s ability to achieve stability and independence.

Responsibilities

  • Street Outreach & Engagement Conduct daily street outreach to engage individuals experiencing homelessness.
  • Build trusting, respectful, and supportive relationships with clients in street-based settings.
  • Provide consistent and compassionate engagement to individuals with serious mental illness and co-occurring substance use disorders.
  • Case Management & Care Coordination Serve as the primary case manager for assigned clients, developing and implementing individualized service plans.
  • Conduct assessments, including the VI-SPDAT, to determine appropriate interventions and service prioritization.
  • Coordinate services across healthcare, behavioral health, housing, and community-based providers.
  • Collaborate with Lazarus Program partners to support client progression toward housing placement.
  • Housing Navigation & Client Support Support clients in obtaining permanent supportive housing through coordinated efforts with Continuum of Care partners.
  • Assist clients with obtaining identification documents, benefits (SSI, veterans’ benefits, etc.), and other essential resources.
  • Provide referrals and connections to employment, vocational rehabilitation, and supportive services.
  • Teach life skills including budgeting, money management, and independent living skills.
  • Client Support & Skill Development Provide supportive counseling, problem-solving, and guidance to assist clients in achieving personal goals.
  • Support development of social, interpersonal, and community integration skills.
  • Assist clients in accessing transportation and attending appointments, including driving clients as needed.
  • Documentation & Compliance Maintain accurate and timely case notes and documentation in accordance with organizational policies and procedures.
  • Participate in team meetings, case conferences, and treatment planning sessions.
  • Ensure compliance with program requirements, regulatory standards, and organizational policies.
