About The Position

The Case Manager provides pre-release and re-entry services to individuals involved in the criminal justice system who have mental health and/or substance use disorders. This role ensures continuity of care by supporting clients during incarceration and following them into the community upon release. The Case Manager delivers short-term case management, connects clients to essential resources, and advocates for successful reintegration, with a focus on housing stability, treatment access, and overall well-being.

Requirements

  • Demonstrated knowledge of the criminal justice system, offender re-entry processes, and community resources, including housing services, client assessment, and crisis intervention.
  • Associate or Bachelor’s degree in a mental health-related field required; minimum of two (2) years of experience in human services preferred, with a focus on mental health.
  • Relevant licensure or credentials preferred (e.g., social work, counseling, or social work assistant), or actively working toward licensure.
  • Valid driver’s license with the ability to operate a vehicle and maintain current auto liability insurance coverage.
  • Must have a satisfactory driving record and successfully pass a background check.

Responsibilities

  • Provide pre-release engagement and post-release case management to support successful community reintegration.
  • Conduct client assessments and coordinate services, including housing, mental health, substance use treatment, medical care, and other supportive resources.
  • Assist clients with obtaining benefits and entitlements such as SSI/SSDI, Medicaid, and other support services.
  • Develop and maintain partnerships with community organizations, service providers, and criminal justice stakeholders.
  • Deliver crisis intervention and support, including assessment of client needs and functioning.
  • Advocate on behalf of clients within community systems to ensure access to necessary services.
  • Maintain accurate and timely documentation, case notes, and program data in compliance with agency and funder requirements.
  • Participate in team meetings, case reviews, and quality improvement activities.
