About The Position

Responsible for providing case management services to seriously mentally ill and/or substance use adults who are currently or have been previously hospitalized and/or in the jail systems. Position will be responsible for completing and implementing case management plans with the goal of preventing future hospitalizations and/or incarcerations through linking recipients with community resources and continuing behavioral health care. Will be responsible for maintaining appropriate records. Some travel may be required within the catchment area.

Requirements

  • Knowledge of psychiatric and substance use disorders in the adult population and/or knowledge of addiction as a primary/secondary illness.
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention, and interventions common to acute psychotic individuals and/or individuals with substance use issues as well as , to non-violent crisis intervention practice.
  • Knowledge of available community resources.
  • Knowledge of educational methods appropriate to adults with sever and persistent psychiatric disorders
  • Ability to design, implement/administer individualized behavioral services in regard to case management services.
  • Ability to provide services in all facilities and other locations as deemed necessary.
  • Must be motivated and able to make quick and accurate decisions. Must be able to meet and exceed expectations. Must be able to organize and perform all duties with minimum mistakes and without constant supervision.
  • Ability to effectively present information and respond to questions from groups of supervisors, managers, and/or the general public in an effective and understanding manner, to promote quality of care.
  • Ability to maintain unit files to include but not limited to recipient charts, case management documentation, etc. in accordance with the requirements of SpectraCare policies and procedures, Alabama Administrative Code, CMS and all other applicable regulating and contractual entities.
  • Ability to attend to detailed assignments and follow through with task until completion.
  • Ability to perform all duties in a safe manner, without endangering any employee or asset.
  • Working knowledge of business English, spelling and math.
  • Working knowledge of office practices and procedures.
  • Skill in keying rapidly and accurately.
  • Ability to provide excellent verbal and written communication skills.
  • Ability to communicate with recipients, families, and/or other agencies regarding case management services.
  • Ability to adhere to health and safety regulations.
  • Ability to adhere to duties assigned by Supervisor and/or other designees.
  • Ability to operate office machines such as computers, copier, fax machines and/or other equipment or programs.
  • Ability to make minor decisions in accordance with rules and regulations and apply these to work problems.
  • Ability to understand and follow moderately complex oral and written instructions.
  • Ability to maintain confidentiality under HIPAA law and 42 CFR
  • Ability to establish and maintain effective working relationships with other employees, all levels of organizational staff and the public.

  • Ability to travel within catchment areas via personal transportation and/or system transportation as required.

  • Ability to pass a drug screen when given to include but not limited to pre-employment, random, post-accident, suspicion, etc.
  • Ability to maintain professional boundaries with recipients i.e., not crossing professional boundaries to include but not limited to fraternization.
  • Complete initial new hire training.
  • Complete required on the job training (OJT) training by the department.
  • Obtain proper certifications needed to perform job duties.
  • Complete annual training as required by the Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
  • Complete Care Coordination / Case Management Training as required by the Alabama Department of Mental Health
  • Complete First Aid/CPR upon hire and working alone with clients; and continuously thereafter maintaining current certification.
  • Bachelor’s degree in psychology, social work, human services and/or related field.
  • Must have a valid driver’s license with clean MVR.
  • Must maintain automobile liability insurance.
  • Willing to complete Case Management training upon hire.
  • Must meet insurability requirements to drive a system vehicle (21 years of age).

Nice To Haves

  • Prior case management experience in a mental health/substance use setting.

Responsibilities

  • Coordinates and provides case management services to recipients in the “Stepping Up” program.
  • Represents the needs of the persons admitted into the “Stepping Up” program and insures quality services are provided.
  • Completes needs assessments as required.
  • Develops and implements case management service plan for recipients.
  • Represents the needs of persons with serious mental illness and co-occurring substance use disorder issues.
  • Assist with maintaining recipient records in a professional manner and in accordance with all SpectraCare policies and procedures, Alabama Administrative Code, CMS, and all other applicable regulating and contractual entities.
  • Courteous and respectful towards recipients, visitors, and co-workers.
  • Performs other duties as assigned to include but not limited to:
  • Fulfill duties of Nurse Delegation Program.
  • Regular attendance with timely arrival and departure according to scheduled hours.
