About The Position

The Change Realization Consultant is an individual contributor that is skilled in change leadership and partners with the Senior Change Realization Consultants and AVP of Change Realization in facilitating and sustaining change. This position is responsible for planning, implementing, and overseeing change initiatives with an organization. This roles focuses on the people side of change – ensuring adoption and minimizing resistance to changes in processes, systems, technologies and structures. The Change Realization Consultant is responsible to support fellow change colleagues with organizing events such has change implementation planning sessions, education, and the enterprise-wide community of practice.

Requirements

• Bachelor’s Degree in Healthcare Admin, Education or related field
• 2 years of experience leading and supporting teams through change initiatives or within an organization development field which may include learning and organization development
• Familiarity with change internal or industry standard change management methodology like CAP, Prosci, SEF, etc.
• Change Management certification preferred

• Proficiency with Microsoft 365
• ICGB - Certified Lean Six Sigma Green Belt - International Association for Six Sigma Certification
• CCMP - Certified Change Management Professional - PROSCI
• GE Master Change Agent - General Electric

Responsibilities

• Organize and support change initiatives such as education and change implementation planning session.
• Co-facilitate education programs
• Integrate change realization activities into project plans.
• Coordinate and support change function project plan activities
• Create deliverables for communications, roadmap, education and resistance management..
• Create calendar of change events.
• Researches best practices and supports in the development of change leadership materials for organization-wide programs
• Data and Reporting: Track feedback and success metrics.
• Report on adoption, utilization, and proficiency.
• Track and maintain change initiatives, change leaders, and change agents
• Engagement: Models Change Agent behaviors and habits to instill change culture throughout the organization.
• Build strong relationships and communications to position process change work as a key enabler to strategy to execution efforts.
• Model Jefferson values and habits in change efforts
