Position Summary The Health Information Management Specialist is responsible for maintaining, processing, and reviewing paper and electronic patient health records for hospital and clinic settings. This role ensures records are handled accurately, timely, and in compliance with HIPAA, hospital policy, and all applicable regulations. The specialist also reviews records for documentation deficiencies and supports providers in completing records before they reach delinquent status.

Requirements

  • HS Diploma/GED
  • Knowledge of medical terminology preferred.
  • At least 1 year of HIM/healthcare experience required.

Responsibilities

  • Retrieve, compile, process, and maintain paper and electronic health records for hospital and clinic patients in accordance with administrative, ethical, legal, and regulatory requirements.
  • Monitor incoming documents daily and handle them promptly according to established policy.
  • Maintain the strictest confidentiality of protected health information (PHI) in compliance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Process and respond to routine requests for patient information in accordance with Three Crosses Regional Hospital policy and applicable local, state, and federal regulations.
  • Validate and ensure that HIPAA and PHI-related forms are current, accurate, and complete.
  • Analyze inpatient, day surgery, observation, and outpatient records for documentation deficiencies.
  • Perform concurrent and retrospective review of patient records to support timely physician completion before records become delinquent.
  • Organize and maintain health record files and logs related to patient diagnoses, treatment, admissions, discharges, and daily census changes within the EMR.
