About The Position

As the Clinical Intake Supervisor, you will assess clients for appropriate placement into SAHLC treatment programs by completing Biopsychosocial, ANSA, SOGS, and health risk screening forms. Develop an Initial Treatment Plan for clients. Assures all releases are filled out appropriately and have the proper signatures. Transfer clients to the primary clinician and case managers as needed. Coordinate with external referral sources to accommodate the appropriate service needs of clients. The Clinical Intake Supervisor will ensure that the Mission of The Salvation Army is effectively carried out.

Requirements

  • Master’s Degree or Licensing Track (License Mental Health Counselor LMHC or License clinical Addiction Counselor LCAC), LCSW (Licensed Clinical Social Worker).
  • Position requires a background check to be completed, and all background check results will be reviewed.
  • Two years of office experience
  • Must have a valid driver’s license and maintain The Salvation Army Driver’s qualification standard; must complete Safe From Harm training within the first 90 days of employment. Must be working towards or currently have CADAC certification
  • Able to speak, write and understand English in a manner sufficient for effective communication with leadership, field personnel, and clientele.
  • Computer proficiency with Microsoft products and ability to learn electronic reporting systems.
  • If in recovery, must demonstrate a minimum of 2 years sobriety including emotional and social stability

Responsibilities

  • Conducts intake/admission including collecting basic demographic and background information, completing SOGS and TCUDS, comprehensive needs, abilities, strengths and preferences, drug use history, and orientation to SAHLC, services and expectations.
  • Conduct assessments for Withdrawal Management clients within 48 hours of admission if the client is medically stable
  • Conduct assessments for external clients entering Residential Treatment, 3.1 Services, Transitional Housing, and Intensive Outpatient
  • Participate in community engagement activities to promote services, facilitate assessments, and build programming models
  • Enrolls and re-enrolls eligible clients in the DMHA Supported Client or other funding programs by completing the appropriate enrollment forms
  • Develop and Initial Treatment Plan to initiate treatment for the client
  • Completes the Biopsychosocial with the client
  • Completes an action sheet regarding placement of the level of care
  • Completes a Clinical summary pertaining to clients’ history and presenting problem
  • Utilizes DSM V to accurately diagnose the client based on the criteria given in the interview
  • Staff all cases in a weekly multi-disciplinary team meeting to assess the appropriate treatment level of care and diagnosis
  • Maintains client confidentiality according to Federal regulations and The Salvation Army standards
  • Provide initial and on-going assessment and clinical training for staff
  • Provide coverage in absence of the Clinical Director as assigned
  • Facilitate Clinical Leads meeting on a weekly basis to support compliance with accreditation and contractual standards
  • Attends staff meetings, Safety Committee, Quality Improvement Committee, departmental meetings, seminars/conferences, and in-service training programs as scheduled
  • Maintains all certifications by meeting certification requirements
  • Enter all the required client information into the EMR system
  • Other tasks as assigned by leadership
